Classification of Unsafe States
1. Lack of protective, insurance, signal and other devices or these devices are defective
(1) No Protection:

- The excavation of the tunnel is improperly supported;
- The explosion-proof device is improperly installed;
- The safety distance for logging and skidding operations is not enough;

- The live parts of electrical installations are exposed, etc.

2. Defects in Equipment, Facilities, Tools & Accessories
(1) Improper design and unsuitable structure for safety requirements:

- The access door blocks the line of sight; the brake device is defective;
- The safety distance is not enough;
- The barrier net is defective; the workpiece has sharp burrs;
- The facilities have sharp chamfers, etc.

- Ropes for lifting heavy objects do not meet safety requirements etc.

(2) The protective equipment and appliances used did not meet the safety requirements.
4. The environment of the production (construction) site is not good
(1) Poor Lighting: Insufficient illumination; blurred vision because of smoke and dust in the work site; too strong light;
(2) Poor Ventilation: no ventilation; low efficiency of ventilation system; wind flow short circuit; blasting operation when power or wind is off; gas emission does not reach the safe concentration of blasting operation; gas exceeds the limit, etc.;
(3) Narrow workplace;
(4) The work site is messy: unsafe stacking of tools, products and materials, etc.;
(5) The configuration of traffic lines is unsafe;
(6) The design or configuration of the operation process is unsafe;
(7) Slippery ground: there is oil, ice and snow or other slippery liquids or objects on the ground;
(8) Unsafe storage method;
(9) Inappropriate temperature and humidity of the environment.
List of Hidden Dangers of Common Equipment & Facilities
- There is no protective cover for the operating part of the mechanical equipment (wheel, shaft, gear, etc.);
- There is protective cover for manual grinders;
- The height of the railing is less than 1.05 meters or the strength is not enough;
- The angle of the ladder is too steep, greater than 75°;
- Lifting equipment limit failure (main and auxiliary hooks, protective doors, etc.);
